From the resources page, you can navigate to the File of Life page and download a PDF to list your medical information & history to help first responders.

Choose one of the four forms to suit your needs.

After filling out the form, a common place to put it is on the refrigerator or taped to the inside cupboard of where you keep your medications.

Agencies: download these forms and encourage your constituents to have them completed for both your & their future convenience. One of the full page variations is brandable with a space left for your letterhead at the top.
